YOU DO NOT HAVE TO PROVIDE YOUR NAME AND EMAIL ADDRESS. However, if you do not provide this identifying information, you will not be entered to win the prize detailed below.
PURPOSE OF SURVEY
The department is trying to identify areas of weakness and strength in its current research mentorship and advising for students. It is important that you be as honest and thoughtful as possible in responding to this survey because this information is being collected for the purpose of improving our department. As noted above your responses will remain anonymous and your name and email address will not be connected in any way to your responses.
Incentive Raffle
We understand that students are busy, and want to express our appreciation for you taking the time to improve the department. For this reason students who provide their information below and complete the following survey will be entered into a prize raffle.
The winners of the raffle will be identified by random selection.
There will be three (3) random prize winners.
The prize: an academic book OF YOUR CHOICE valued at $100 or under. Along with your name and email address, we also ask that you provide the TITLE and ISBN of an academic book that you would like us to buy for you. The winners will receive the book they selected.
